Correct
Accurate data, correct patient, correct spelling. Wrong details anywhere in the chart undermine the credibility of everything else in it.
Chronological
Time-stamped and in order, with any gaps explained. Entries that are out of sequence — or a late entry with no explanation — read as suspicious even when nothing was hidden.
Concise
Facts only, no filler. Padding a note with vague or emotional language dilutes the objective findings that actually protect you.
Consistent
Matches what the rest of the care team documented and what was ordered. Contradictions between notes are one of the easiest things for opposing counsel to exploit.

Excellent review of things that I’ve charted and different ways to say them. Easy read. Well done and should be read by all nurses if they have one year of experience or 20 years.
Good value for starters but could be better
I wish it had more in-depth examples of documentation in critical situations; also assessment-wise grading like “scant”, “moderate” etc it would’ve been nice to see visuals to understand the grading. Price is ok because you would probably pay more for a comparable workshop. Some stuff gets repeated multiple times, and overall the book could be probably half the number of pages. Some typos here and there in terms of formatting
